Investigation Summary
At approximately 10:45 a.m. on January 16, 1997, Employee #1 was in a wooden box (personnel cage) that was being elevated by a Case 585C forklift to a second-story stairway landing so he could attach the last metal bracket to two beams. As Employee #1 went up, the forklift was also being moved forward to get closer to the building. The forklift jerked and shuddered, and the box fell. Employee #1 and the box struck the second-story stairway landing and then fell approximately 10 ft to the concrete sidewalk. He suffered severe head injuries and was transported to Good Samaritan Hospital in Corvallis, OR.
