Investigation Summary
At approximately 3:05 p.m. on November 3, 1996, Employee #1, a plant electrician at Weyerhauser Containerboard in Springfield, OR, had just picked up an 85 lb roll of electrical wire from the shop and was on his way out of the building. He walked about 16 ft into the driveway before he was struck and crushed by a Hyster forklift that was approaching from his right side. He was run over and killed instantly.
