Employee injured when run over by forklift

At 8:05 p.m. on August 7, 1995, Employee #1 was walking back to her work station through an aisle lined by rows of stacked plywood. A Jitney forklift operator backed out of the stacked plywood and proceeded to back down the aisle while looking over his right shoulder. The backing forklift struck Employee #1 in the back, knocking her to the ground. The left rear wheel of the forklift then ran over her right leg, removing flesh and tissue from her hip to her ankle. Employee #1 was hospitalized.
