Investigation Summary
Employee #1 was operating a Link Belt 2700 track hoe at Multnomah Falls, OR during freezing rain and snow. The snow had mixed with Multnomah Falls Creek water to make a very slushy consistency. The slush and freezing rain had caused the stream to back up into the lodge and to run over the railroad tracks. Employee #1 was working downstream from the tracks, attempting to clear the snow and ice so the creek would flow normally. He positioned the track hoe so that its tracks were parallel with the creek. Employee #1 then swung the track hoe arm around to his left. He was apparently reaching out when the track hoe tipped over, submerging the cab in the creek. Employee #1 drowned before he could be rescued.
