Employee injured when scissor lift tips over

At approximately 1:00 p.m. on August 21, 1995, Employee #1 and a coworker, of Skyline Sheet Metal, Inc., were attaching a "hat channel" to a metal awning in preparation for installing the sheet metal covering. The two employees were working from a scissor lift extended approximately 18 ft above the adjacent floor level. The lift was positioned in front of a floor hole measuring approximately 7 ft by 8 ft and approximately 12 in. deep. The front tires of the scissor lift entered the floor hole, tipping the lift over and throwing Employee #1 to the concrete floor. The coworker was able to escape falling by climbing onto the awning. Employee #1 was hospitalized with multiple bruises and contusions.
