Investigation Summary
At 11:00 a.m. on August 28, 2017, Employee #1 was in the production area, chopping fresh spinach using the OVI slicer, identified as Machine C-1. The employee opened the bagged spinach and poured the contents of the bag into the chopper/slicer. The spinach was drawn by a fast-moving conveyor to the chopping/slicing blades at the end of the conveyor before being deposited into a bin just outside and below the chopping/slicing portion of the machine. Employee #1 had poured the final bag of spinach into the conveyor. He then reached into the machine with his left hand to push the spinach that was stuck to the sides of the machine into the conveyor. The employee misjudged the hazard, as well as speed of the conveyor, and his hand came in contact with the conveyor. The employees left hand was drawn into the chopping/slicing blades, amputating his left middle finger at the first knuckle and lacerating his left index finger. Employee #1 was taken to a nearby hospital where he was treated and released.
