Investigation Summary
At 3:09 p.m. on October 13, 1995, Employee #1 cut a 43 in. by 96 in. staircase opening in the second floor of a residential structure under construction. He then went to move a pre-made partition wall into place and stepped into the opening. Employee #1 fell approximately 24 ft to a concrete basement floor and sustained a fractured skull and back injuries. He was flown by Med-A-Vac to Fairfax Hospital's shock trauma unit for treatment of his serious, life-threatening injuries.
