Investigation Summary
At approximately 11:53 a.m. on April 5, 1996, Employee #1 was lowering and removing H-beam supports from under the second story floor of a house. One of the beams rolled and the southwest end dropped 8 feet to the first floor. It struck Employee #1 in the head, killing him instantly.
