Investigation Summary
On December 2, 1998, Employee #1 was climbing an improperly constructed fixed ladder. The five rungs were 16 in. apart, and clearance on the climbing side of the ladder was blocked by an overhead stainless-steel pipe that was 16 in. away from the face of the ladder. Employee #1 apparently struck the pipe while he was climbing the ladder and sustained a head injury that rendered him unconscious. He fell feet first, 7 ft 10 in., rolled back, and struck the rear of his skull for a second head injury. Employee #1 was transported to the hospital, where he died at 2:30 p.m. that same day. He had no health problems, and his heart was used in a donor program.
