Employee killed in fall from ladder

Employee #1 was using a metal portable extension ladder to obtain a hat rack from a 9 ft 3 1/2 in. high shelf. Boxes weighing 50 to 75 lb fell over on the ladder, causing the employee to fall from the ladder onto the concrete floor. He was killed.
