Investigation Summary
At 11:30 p.m. on September 18, 2017, Employee #1 was participating in a PIT training session, learning to operate a powered pallet jack, Raymond Model 8410, ID number 420PE, and Serial number 841-15-26070. The employee was operating the powered pallet jack on the aisle near the maintenance area at the rear of the warehouse when she lost control. She suffered a fractured leg as a result of the impact with the wall of the warehouse. Employee #1 was transported to a local hospital where she remained for two weeks.
