Investigation Summary
Employee #1 was on a scaffold unloading concrete blocks from a pallet that had been hoisted up to the scaffold platform by a forklift. The pallet of blocks was banked by plastic shrink wrap, which Employee #1 was removing. He was leaning out over the edge of the scaffold, supporting himself by holding onto the pallet of blocks, when one of the blocks dislodged. Employee #1 fell 20 to 22 ft off the scaffold onto the asphalt pavement. He had removed the scaffold guardrail to facilitate off loading.
