Employee injured when struck by falling concrete form

Employee #1 was a connecting steel worker, working below the roof area of a barracks building under construction. Above him, a 10 ft high by 40 ft long by 8 in. wide metal form was being lowered on top of another form already in place, in preparation for pouring a concrete wall. The form was top-heavy and it began to tilt. The bottom then slipped off the lower form and struck Employee #1. He suffered bruises and a sprained back.
