Event: 12/19/1995
Employees exposed to ammonia gas when end-cap ruptures

At approximately 12:15 p.m. on December 19, 1995, an end-cap on the suction side of an ammonia refrigeration system ruptured. Ammonia vapors were pulled into the plant by intake fans located on the roof near the rupture. The building was evacuated and an emergency response was initiated to stop the leak. Approximately 35 employees, including Employees #1 and #2, were treated as needed for breathing problems, but no one was admitted to the hospital. Citations were issued for process safety management deficiencies and for violations of the respirator standard.
