Investigation Summary
At approximately 10:30 a.m. on June 9, 2017, Employee #1 had just delivered steel sawhorses used to support the rebar during the placement on a decking. He was walking near forklifts used to transport and relocate the equipment holding the rebar and sawhorses to another part of a yard. The equipment was secured to a sling and suspended on a hoisting apparatus that was connected to forklifts. A coworker used a telescopic material handler, as a hoisting apparatus, to move the equipment to another location at the yard. The coworker hooked the equipment to the attachment point on the lift via the sling and was traveling in a west direction. Employee #1 was walking towards the moving equipment. He walked into the travel path of the forklifts to assist with controlling the rotating of the sawhorses. The front right wheel of one of the forklifts struck and crushed his left foot. Employee #1 was transported to a medical center, where he received treatment and was then hospitalized.
