Investigation Summary
At 10:57 a.m. on September 6, 2017, Employee #1 was cleaning the Risco meat processing machine. The employee reached into the tray of the machine with his hand. At that moment, his leg hit the self- actuating lever, enabling the machine to cycle, and causing the machine blade to engage. Employee #1 suffered a partial amputation of his ring finger, and was hospitalized for treatment.
