Investigation Summary
At 2:45 p.m. on July 13, 2017, Employee #1 was on the top of a injection molding machine, prying open the plates that were stuck. After prying the plates open, the employee began to climb down off of the injection molding machine. As he was climbing down, Employee #1 grabbed the frame on top of the machine. The safety bar was in motion, and managed to pinch the employee's fingers between the safety bar and the frame of the machine. Employee #1's fingers were crushed, requiring multiple surgeries and hospitalization for approximately 3 days.
