Employee amputates fingers in panel saw

At approximately 9:45 a.m. on March 30, 1995, Employee #1 was using a horizontal panel saw to trim a door. When he reached behind the blade to catch the 1/4 in. fall-off, he struck his left thumb and index finger against the blade, amputating them.
