Investigation Summary
Two employees received lacerations to their hands when they made contact with the blade of an operating aluminum fan. The fan was approximately 36 in. in diameter and the fan guard was missing at the time of accident. On the day of accident Employee #1 was working in the kitchen as a dishwasher. At approximately 1:00 pm the guard came off the fan. Employee #1 put the guard back on the fan and continued working. At approximately 2:30 p.m. the guard came off again. Employee #1 put the operating fan under the dishwasher shelves without the guard and continued with his duties. At approximately 3:00 p.m. Employee #1 grabbed a water bottle for his break and dropped it on the floor under the dishwasher shelves. As Employee #1 reached under the shelves to retrieve the water bottle and he made contact with the blades of the operating fan resulting in the laceration of the left index and middle finger. Employee #2 came to assist Employee #1, and reached under the shelves to unplug the radio to reduce noise levels around the accident scene also made contact with the blade of the operating fan. Employee #2 received lacerations to her right thumb. Both employees suffered lacerations and were treated for their injuries, but were not hospitalized.
