Investigation Summary
At approximately 3:00 a.m. on August 17, 2000, Employee #1 was working on a table in close proximity to the bar when the phone rang which was located near the end of the bar. He lunged from the table to answer it and in the process slipped and fell on the floor. He sustained three fractures and a dislocated right ankle. Employee #1 was hospitalized.
