EMPLOYEE SLIPPED, FELL, AND BROKE HAND

At approximately 12:00 p.m. on January 14, 1998, Employee #1 was on a field trip when he slipped and fell, breaking his right hand. Employee #1 was walking on a slight slope on wet grass. Employee #1 slipped on the wet grass and fell backwards. Employee #1 put his hands out to break the fall and broke his right hand. Employee #1 was taken to the hospital and treated and released.
