Investigation Summary
On November 9, 1997, Employee #1 was in the process of cleaning the granulator system. Employee #1 was found by a coworker in an injured state on the floor. He was given first-aid and was then transported by ambulance to Mercy Hospital in Bakersfield, where he was admitted for treatment for five days. Employee #1 does not know how he was injured. It is possible that Employee #1 fell through an elevated opening in a 4-foot by 3.5-foot work platform located 10-foot above the floor. Employee #1 also could have been attempting to move a forklift and lost his footing while attempting to enter the forklift. Since Employee #1 does not remember how he was hurt and there were no witnesses to the accident, it was not possible to determine the cause.
