Employee's fingertip amputated in chocolate pump machine

Employee #1 was an EPSU team member for the Nestle Ice Cream Company, Bakersfield, CA. His job was to clean up and adjust equipment in the ice cream plant. He put his right index finger into an operating Wahisha chocolate pump on drumstick line #10 to check it for dryness. His fingertip was amputated. Employee #1 failed to follow standard procedures for lockout/tagout while cleaning the pump.
