Investigation Summary

At approximately 2:45 p.m. on April 17, 1997, Employee #1, a truck driver with Vecta Transportation Systems, was picking up some freight at a customer's facility in Bakersfield, CA. He was standing in the back of his truck while a forklift operator from the facility helped him load the freight. Employee #1 placed a 2 by 4 in front of a pallet so the forklift operator could place his forks against the board and push the pallet forward into the truck. The operator engaged the wood before Employee #1 could move his hand. His left ring finger became caught between the board and the pallet, and was amputated at the first knuckle.
