Investigation Summary
At approximately 12:00 p.m. on May 13, 1997, Employee #1, who worked for Robinson Presioso, Inc., was on a 15 ft 6 in. high extension ladder. The ladder slipped down the wall, carrying Employee #1 with it. He fractured his right upper leg and was in the hospital for eight days.
