Investigation Summary
At approximately 8:00 a.m. on December 16, 1996, Employee #1, of A.J. Diani Construction, Inc., was walking on the roof of a building. He slipped on the plywood sheeting and slid approximately 12 ft down the sloped roof on his feet, as if he were skiing. Employee #1 went off the edge of the roof and fell approximately 12 ft to the ground, landing on his feet. He sustained a fractured lower right leg and a fractured left heel.
