Employee injured in fall with ladder

At approximately 11:45 a.m. on October 12, 1996, Employee #1, of Premier Painting & Decorating, was standing on the third step from the top of a 10 ft portable stepladder, masking off around the top of an approximately 14 ft high sign. The ladder had been placed on sloping ground, and it was not tied, blocked, or otherwise secured to prevent it from being displaced. As Employee #1 reached out to mask above the sign, the ladder shifted and came away from the wall. The ladder and Employee #1 fell to the asphalt. He sustained a broken right wrist and elbow, and back injuries.
