Investigation Summary
At approximately 5:00 p.m. on June 19, 1996, Employee #1 and the owner of the Koster Company were setting up a concrete paving machine for laying concrete the next day. Due to an error in grading, the machine was against an 18 in. high earth bank and could not move forward. Employee #1 and the owner tried to adjust the width of the tracks by moving the track turnbuckles while the machine was in operation. Employee #1 slipped off the bank, and his leg became caught between the machine and the track. He sustained a crushed left big toe and a fractured right fibula. The owner ran to the controls and bucked up the machine, freeing Employee # 1.
