Employee Is Burned When Flammable Liquid Ignites

At approximately 9:30 p.m. on November 14, 2008, Employee #1, a diesel mechanic, was cleaning a metal work bench in the truck maintenance garage. He was using alcohol from a one-gallon container and a rag to clean the work bench, when a torch that was on the bench fell. The torch created a spark that ignited the alcohol on the work bench and the alcohol in the container. Employee #1 suffered burns from the fire. He was transported to Glendale Memorial Hospital and then transferred to Grossman Burn Center where he was treated for second- and third-degree burns and was hospitalized for 12 days. Further investigation revealed that Employee #1 had not been trained on the safe use and hazards of working with alcohol or flammable liquids.
