Employee's Head Is Injured in Fall from Ladder

At approximately 9:00 a.m. on June 22, 2010, Employee #1, an installation apprentice, was on an 8 ft wooden A-frame stepladder performing improvements on the HVAC system of the 12th floor. He was standing on the second step below the top cap. The ladder wobbled, and he and the ladder fell to the floor. He did not recall any deficiencies with the ladder. He was hospitalized for eight days and treated for skull and brain injuries.
