Investigation Summary
On October 9, 2012 Employee #1 was making electrical connections for a burn in box. To perform the electrical work, Employee #1 was working with a energized panel. Employee #1 stated that he was working energized because he could not turn off the power because it would turn off power to the company's operations. Employee #1 was not wearing any protective equipment, and made the electrical connections while the panel was energized and completed the connection work. Employee #1 used a regular paint brush with a metal band to brush out metal chips that were inside the panel. The energized parts arched because of the metal band on the paint brush. Employee #1 suffered serious burn injuries and was hospitalized at a local medical center for several days.
