Investigation Summary
At approximately 9:15 a.m., on May 2, 2012 Employee #1, a packer of Herbthyme Farms Inc was assigned to the production line #8 to collect clamshell containers coming out of the labeling machine and place them in boxes. Employee #1 noticed that the bottom label was not sticking to the clamshell container. Employee #1 went over to Employee #2 to ask for a help but Employee #2 was helping out another employee on the production line #9 and could not go with Employee #1 immediately. Employee #1 came back to the production line #8 and she tried to unjam a label from the bottom labeler with her left hand without stopping the machine. Employee #1's left index finger got caught in the machine and a part of her finger was severed. Employee #1 was taken to the office and the HR Manager transported Employee #1 to Immediate Medical Center. Shortly Employee #1 was referred to Western Hand Center in Downey where she underwent a surgery for a partial amputation of her left index finger.
