Investigation Summary
At approximately 7:00 p.m., on January 18, 2011, Employee #1 of Wagner Rubber Products Inc. was operating press number 24, which required the cleaning of the injection cavity to remove the excess flash every third cycle. Employee #1 was operating the press in automatic mode which required the operator to press the two control buttons on the two-hand control panel only once to complete each cycle. Employee #1 operated press number 24 with the side door kept open. After completing one cycle, Employee #1 took out the molded products from the injection press and started another cycle by pressing the two control buttons on the two-hand control panel. Employee #1 then proceeded to clean the injection cavity. Employee #1 accessed the injection cavity through the open side door and by standing on the top step of a step ladder. Employee #1 reached in with her right arm to clean out the excess flash from the cavity. As Employee #1 was pulling her arm away, the injection nozzle came down and it pulled in her right hand. Employee #1 called a coworker who was working at press number 3. The coworker came over to help but he was not familiar with the machine. Employee #1 told the coworker to switch the operation mode to manual by turning the key. Employee #1 called out for another coworker and he came over to help. This coworker was familiar with press number 24 and he was able to raise the injection nozzle and release Employee #1's hand. Employee #1 picked up her right hand from the cavity and came down from the step ladder. Other employees came over to help Employee #1 and a couple of employees summoned emergency medical services. Shortly thereafter the fire department and the paramedics arrived at the site and Employee #1 was transported to Harbor UCLA Medical Center where she was hospitalized for 5 days for an amputation of right hand. The press number 24 was manufactured by LWB Steinl and its model number and serial number were VRE 2500/1000 and 272440 respectively. The side door was equipped with the safety switch and it was not working at the time of accident. Employee #1 was wearing heat resistant sleeves, cotton/polyester blend knit gloves and safety goggles. The accident was caused because the injection molding press was not deenergized or the power source of the machine was not disengaged or the moveable parts were not mechanically blocked or locked out to prevent inadvertent movement during the cavity cleaning operation.
