Investigation Summary
At approximately 9:30 a.m. on September 13, 2008, Employee #1, a Meat Cutter, and a coworker had finished cutting approximately 15 oxtails in approximately 10 minutes. Employee #1 and the coworker were both using butcher knifes (a 16-in. fine toothed butcher's knife) in a chopping motion to cut oxtails. The employer did not provide hand protection to meat cutters using butcher knives. The oxtail Employee #1 was holding with his right hand slipped away, subsequently causing his right hand to enter the path of the knife. Employee #1 was hospitalized.
