Investigation Summary
Employee #1 was working for a company that is engaged in party arrangement and equipment rental. Employee #1 was carrying two chairs on his shoulder and was climbing an extension ladder to store them on the top shelf. Employee #1 was standing on the third rung from the top and tried to put the two chairs onto the top shelf (approximately 8-ft 10-ins. above the ground). The ladder moved to the left and Employee #1 fell approximately 7 ft to the ground. Employee #1 was organizing party supplies. There are steel frame shelves in storage area.
