Employee Injured In Fall From Manlift

Employee #1 arrived at work at approximately 4:00 p.m. and parked cars from 4:00 p.m. until 10:30 p.m., just 30 minutes before the end of the shift. Employee #1 parked the last customer's car on the lower level at approximately 10:30 p.m. and took the manlift (Elevator ID Number 00043347) to the top level. Employee #1 was not trained on the hazards of riding a manlift. When Employee #1 got to the top floor he put his right foot on the floor that was slippery. Employee #1 fell head-first approximately 15 ft. Employee #1 was the only one riding the manlift at the time of the accident. Employee #1 sustained several injuries requiring staples in his head, back, and arms and was hospitalized.
