Investigation Summary
On April 11, 2007, Employee was a performing his normally assigned duties as a maintenance operator. Employee #1 stated that he was asked by the lead person to repair the conveyor for the rice cake cutter. Employee #1 stated when he went to work on the equipment he saw that it was not working or moving. Employee #1 removed a guard which covered the belt and pulley drive for the conveyor and the belt was not moving when he removed the guard. Employee #1 made an adjustment of the tension of the belt by turning the tension screw. When Employee #1 turned the screw, the belt and pulley started moving. Employee #1's left hand was in the area of the pulley nip point. Employee #1 attempted to remove his hand, but he was unable to remove his thumb. Employee #1 stated that the conveyor was plugged in, but he did not know whether the machine was on or off when the lead person asked him to work on the machine and when he started to adjust the belt and pulley. Employee #1 had not received lockout and tag out training from the company and is not familiar with a company program. Employee #1's left thumb was amputated at the tip. Employee #1 was treated for his injury but was not hospitalized.
