Investigation Summary
At approximately 10:00 p.m. on March 8, 2007, Employee #1. a waiter, carrying a bamboo try with a small hot pot of soup from the kitchen to a table 30 ft away. He was wearing tennis shoes and the floor was in good condition. As she approached the table, her left ankle slipped, and she fell. The hot soup splashed on her and she suffered second degree burn to her face, and third degree burn to her left arm. Employee #1 was hospitalized.
