Investigation Summary

Investigation Nr: 201168705
Event: 02/27/2007
Truck Driver's Ankle Is Fractured by Pallet Jack Backing Up

At 8:45 a.m. on February 27, 2007, Employee #1 was working as a truck driver for 3 D Delivery. He had picked up a truck at the 3 D Delivery yard at 7:00 a.m., and he arrived at a Delta Cargo facility at 8:30 a.m. to pick up some boxes, parking at door Number 19. There, an employee of Swissport moved a pallet jack toward door Number 19 to deliver some boxes to Employee #1. After receiving the boxes from Swissport worker, Employee #1 stood between the back of his truck and the edge of the loading dock to close his truck's door. Instead of having the pallet jack go forward, the Swissport operator had it go backward, pinning Employee #1's ankle against his truck. Hearing screams, the operator moved the pallet jack away to release Employee #1's ankle. Employee #1 sustained multiple fractures to his right ankle. He was taken to Centinela Hospital, where he was hospitalized for four days.
