Investigation Summary
At approximately 6:05 p.m. on August 27, 2008, an employee and coworkers were assigned to work on the roof openings to install seismic bracing on the exposed roof beams prior to resheathing. The employee stated that while he and a coworker were working the day before, at times the employee was sitting in the opening, straddling the roof beam to gain access to the beams to install the braces. On the day of the accident, the employee and the coworker returned to the roof opening and were beginning the work day by removing the temporary plywood coverings which had been placed the night before. While they worked to remove the covers, the employee fell through the roof opening to the break room floor below. The fall distance was approximately 13 ft. The employee was taken to County USC where he was hospitalized for a back injury from August 27, at 6:30 p.m. until August 28, 2008 at 2:00 p.m. According to the employee, the coworker and the employer's representatives, neither the employee nor the coworker were wearing fall protection while working near the roof opening on the day of the accident and on the day before the accident.
