Employee Is Injured When Struck by Forklift

On May 11, 2007, an employee was a performing his normally assigned duties as a boilermaker apprentice. He was helping to move a welding machine by the use of a Gradall forklift. The welder was rigged to the forks of forklift and a tagline was attached to the welder. While the forklift operator drove the forklift carrying the welder, the employee held the tagline that was attached to the welding machine. During the operation, the tire of the forklift ran over the lower left leg and foot of the employee, while the vehicle was in motion. The employee sustained fractured fibula, toe, and metatarsal with lacerations of the left foot. The employee was hospitalized and treated for his injuries. During an interview, the employee and the forklift operator indicated that they have received health and safety training, including forklift training. In addition, they indicated that the speed of the vehicle was slow and appropriate for the work and work area.
