Investigation Summary
At approximately 2:00 p.m. on March 23, 2006, Employee #1, a pallet carpenter/sorter, was on the ground, when he was run over by a forklift that was backing up. Employee #1 sustained a compound fracture to his right foot and was hospitalized for a week for treatment.
