Investigation Summary
At 6:00 a.m. on August 7, 2017, an employee lubricated a Topper himself. The Topper lift was extended with him in it and the employee began to lubricate the machine. The employee lubricated the Topper machine. He then exited the extended basket to climb down. His foot hit a lever control on the forklift, the mast tilted back pinning him against the roll over protection, and lacerated his abdomen and kidneys.
