Investigation Summary

Investigation Nr: 201115532
Event: 01/15/2003
Employee cut leg on unpadded emergency brake

At approximately 3:15 p.m. on January 15, 2003, Employee #1, a forklift operator, was moving computer parts from an office, thru a storage area, to the outside of the building. After the parts were taken off the forklift, he backed up into the building, down an aisle with citrus storage bins on each side. He got too close to the left side and hit the bins. This caused the forklift's battery cover to be pushed forward. The employee's seat was bolted to the cover, so it also moved forward and his left lower leg hit the unpadded emergency brake. Employee #1 sustained a serious cut to his lower left leg and was hospitalized for treatment at Kern Medical Center in Bakersfield.
