Investigation Summary
At approximately 10:50 a.m. on October 20, 2002, Employee #1, a ginner helper, was using an extension ladder to fix a hole in a 2-in. diameter trash line. He was about 12 ft above the ground. While working on the pipe, the ladder slipped, and the ladder and Employee #1 fell to the floor. He was hospitalized with bruises, contusions, and abrasions.
