Employee's toes amputated when caught in forklift

At approximately 12:00 a.m. on January 20, 2003, Employee #1 was helping a forklift operator move a load. The forklift was a Toyota Model Number 7FGCU32, Serial Number 62214, Type LP. Employee #1 helped him pick up a double pallet of cardboard, 5 ft wide by 9 ft long by 2 ft high. Employee #1 climbed on the forklift roll cage to stabilize the load, which was wobbling because of not being centered and secured. Employee #1 had one foot on top of the roll cage and one foot between the cross bar and the mast. The forklift operator had Employee #1 hold up the load while he tried to get a rope to secure it. As the forklift operator lowered the mast, Employee #1's right foot was caught between the cross bar and the mast. Employee #1 toes were amputated and he was hospitalized.
