Employee's Fingers Are Amputated in Roller Machine

On June 12, 2007, Employee #1 was working for a water systems fabricator. He was forming center sections of water tanks using a roll form machine. He was distracted while feeding the machine and his right hand fingers were crushed by the rollers. He was hospitalized and is fingers were amputated.
