Investigation Summary
At approximately 12:00 p.m. on October 14, 2005, Employee #1, a floor installer, was operating a table saw, Shop Fox brand, 3-horsepower, with 10-in. in diameter blade, Serial Number 3491YB. He was working by himself in the garage located at home of the President of the company. Employee #1 was cutting 2-in. strips from a red oak board, which was 6-in. wide and 3-ft long. The procedure was to cut the board halfway, turn it over and cut it from the other side. Employee #1 pushed the wood through and the blade contacted his hand. The blade seriously lacerated Employee #1's palm, thumb and fingers number 2, 3, and 4 of his left hand. The guard was not on the table saw at the time of the accident. Employee #1 was in the hospital for seven days.
