Investigation Summary
On November 5, 2001, Employee #1 was injured at a single family residence in Chatsworth, CA when he apparently fell approximately 12 feet from an extension ladder atop a one-level scaffold. He and other employees were preparing a garage ceiling for painting. Employee #1 did not recall how the accident occurred. He received a broken left wrist and head trauma which resulted in blindness of his right eye. He received surgery on his left wrist at LA County General Hospital.
