Investigation Summary
At approximately 2:00 p.m. on May 20, 2003, Employee #1, working as a machine operator, sprained his ankle while standing on top of a conveyor belt attempting to unjam some stuck onions. He slipped on the belt when it was still moving. Employee #1 was taken to the hospital where he was treated and released. The facility involved in the accident is a large, onion, processing plant.
